New Patient Intake

Thank you for your interest in becoming a new patient of the Alliance Dental Center.

At this time, we are unable to accept new patients.

However, we do have an intake process you can follow to determine if our office is a ‘good fit’ for you and steps you can take to be considered as a new patient.

There is some very important information you need to understand before you take the steps to apply to be a new patient.

  • The office will only treat MPE Fund patients. You must be a member of the MPE Fund. If you are not sure of your union or if you are eligible for this benefit, you may go to Who is Eligible? or call the MPE Fund at 617.426.4440.
  • The Alliance Dental Center is a general dentistry office focusing on prevention of disease.
  • What the Alliance Dental Center will not do:
    • We do not do implants.
    • We do not replace missing teeth with implants, long-span fixed bridges, or complete dentures.
    • We do not provide cosmetic services.
    • We do not provide extractions, root canals, or orthodontic (braces).
    • We do not see new patients to address an immediate concern and not have the patient commit to a long-term relationship.
  • What the Alliance Dental Center will do:
    • Partner with you to identify your oral health goals to improve your oral health
    • Become your ‘dental home’ so you have a safe comfortable place to receive great care
    • Provide you with minimally-invasive care to reduce your disease and work with you long-term to manage your needs
    • Help you coordinate your care with specialty offices
    • Our patient care advocate works with our patients to help identify barriers to receiving regular care (i.e. fear, anxiety, etc.) and works to address those barriers when possible

After reading the above information carefully, if you still want to be considered as a new patient, you need to print and complete the New Patient Intake Questionnaire form. Once you have completed the form, you can either mail or fax to us at the contact information on the form. We do not accept emails. Upon receipt of your form, you will receive confirmation from ADC with any next steps.  You must allow 21 business days from receipt of your form to receive a confirmation!

Submitting the New Patient Intake form is not a guarantee you will be selected as a new patient.
